All information and records obtained in the course of providing services to the client shall be subject to the confidentiality and disclosure provisions set forth by the HIPAA laws and Title 43, Code of Federal Regulations,
Part 2; and Title 45, Parts 160 & 164. This means that no information will be disclosed to other facilities, law enforcement, or your family members unless you provide a Release of Information (ROI) which will permit disclosure of specific information. Exceptions include:
– You are in danger of harming yourself or someone else
– The disclosure is court ordered
– To medical personnel in the event of a medical emergency
